I understand that in submitting this application, I authorize investigation of all statements contained therein, and authorize Sagepoint Senior Living to make any contacts considered necessary to my participation in the Volunteer Program, such as reference inquiries and criminal records searches. I release all parties, organizations, and institutions from any claims for damages or otherwise by reason of furnishing such information and records.

I understand that I must be at least 15 years of age to volunteer at Sagepoint, and if I am under the age of 18 years of age/or attending high school I will need parental consent.

Upon being offered a volunteer position, I understand that I may be required to provide additional information pertinent to the position for which applied.
